Abstract

This is the virtual presentation on a quality improvement study conducted in fulfillment of the Doctor of Nursing Practice degree requirements. The purpose of this project was to implement a personalized, clinic-based diabetes management program with healthy lifestyles, reduce hemoglobin A1c, and prevent diabetic complications.
